ECON 317 SPRING 2019 : GROUP ASSIGNMENT 4: QALY

This assignment is meant to provide a very rough estimate, in QALY, of the benefit provided by your health intervention. To find this estimate, you must interview at least five different peopleDue to UVic rules relating to ethics and interviews, these people may include ONLY:

· Members of your group.

· Other CURRENT ECON 317 students. (You can ‘trade’ interviews with other groups.)

· Imaginary people (pretend you are someone else and try to answer as they would answer.)

Each person interviewed will be asked to provide quality of life information on a medical condition through a visual analogue scale, a standard gamble and a time trade-off question.

When submitting your assignment, please include:

· Copies of your visual scales, questionnaires and interview scripts (you may use the ones provided, if you wish).

· A table listing the relevant rating (visual scale measurement, standard gamble probability or time trade-off ‘B’ value) for each person interviewed. Do NOT use the real names of the persons interviewed – assign them a code name or index number to maintain their privacy.

· A table listing the QALY calculated from the above, using the techniques from Lecture 7.

· A technical appendix with your QALY calculations.

· A discussion of your results:

· Were your QALY values close together or far apart?

· Did one method (SG or TTO) provide consistently higher or lower QALY values than the other? If so, how do you explain it?

· Do you think your interviewee’s characteristics affected the QALY measures? If so, which ones, and how? (e.g. Age, Gender, Cultural Background, whether they’d experienced the health state before)

· If you were to do this assignment again, what would you change to improve the reliability of your results?

· A summary of your investigation, and what you learned from it. You should also answer the following question: based on your work in this assignment, which QALY measure do you think is the most reliable?

Steps to Performing the Assignment

1. Decide what health condition you will investigate. This should be the main condition that your health intervention treats. For example, if your health intervention helps people find safe, nutritious food, the condition could be acute food poisoning (unsafe food) or chronic malnutrition (lack of nutritious food).

You must specifiy whether your condition is a chronic (lasts forever) or temporary condition. For the math to work, it is important that this condition be preferred to being dead (there are fates worse than death!) Some examples are listed below, but you are free to choose any medical, mental, social or otherwise health-related condition that is of interest to your group.

2. If you choose a temporary condition, make sure that it is worse than heartburn. You will be using heartburn (acid reflux), with a known QALY of 0.82, as your comparison condition in calculations[footnoteRef:1]. 3. Write a description for your chosen condition (and for heartburn, if you chose a temporary health state). This description must be short, easy to understand, and able to convey to the people being interviewed what the condition is like. They must be able to imagine themselves suffering from the condition.

4. Prepare your questionnaires. You may modify the ones provided, or create your own. If you are interviewing people whose first language is not English, it’s fine to have your questions in another language – just be sure to provide English translations in addition to the originals when you hand in your assignment.

5. Conduct your interviews and write down data.

6. Perform the QALY calculations (similar to those in Lecture 7).

7. As a team, discuss the results and write your discussion, summary and conclusion. You can always ask the instructor for help if you need it! Either ask in class, or e-mail willmore@uvic.ca .

8. Edit the report and hand it in.

MARKING RUBRIC:

(30%) DATA COLLECTION, PRESENTATION AND ORGANIZATION: All questionnaires and relevant answers are present and organized in a way that is clear and easy to understand. The privacy of the interviewees is respected, and no identifying information is present.

(30%) QALY CALCULATION AND PRESENTATION: QALY were calculated and placed in an appropriate, clear table. QALY calculations are available in a technical appendix, correct and easy to follow. Any assumptions are stated clearly. Any outside references used are cited appropriately in a bibliography.

(30%) DISCUSSION: All required discussion topics are present. The discussion is well-reasoned, logical and consistent with the data collected. No obviously strange or important feature of the data is ignored. If and when necessary, students made use of outside sources to back up their arguments. These sources are cited appropriately (APA format) in a bibliography.

(10%) SUMMARY AND CONCLUSION: Work is appropriately summarized. The question of which method of QALY estimation is most reliable is addressed logically and in a way consistent with both the data and the discussion.

The reason this is ‘flipped’ is that Outcome S needs to be worse than Outcome H for the math to work – otherwise, the treatment would always be preferred, since both possible outcomes (full health or Outcome S) would be better than Outcome H.

The average Canadian life expectancy is currently about 81 years, so for a 21-year-old interviewee, ‘A’ would be (81 – 21) = 60.

In July 1998 I graduated from high school in my native Havana Cuba. The Government paid careers for you what is good for the part economic but personally is a bit frustrating, since the school appreciates your grades and by the subjects that better grade you has during the school year they assign you a career. The career that they chose for me was accounting I had very good grades in mathematics. I wasn’t happy on my career not I did not felt defined or much less I felt with passion towards.

In September of the same year I started at the school of accounting. I felt he needed more time to choose a career. It was like a fish out of water, continued studying because it was what I supposed to do as young adult. Had not felt that to now that did not exist a love for my career.

Reasons family my aunt took care of my in the evenings, she had that work till night, I did not return to my house when I was finishing my classes I have to be on her work and there I stayed until it ended. My aunt had a very large office, on the second floor of the building filled with balconies and windows, she always had many papers on your desk. My aunt worked for an organization Government that defends the rights of women, at the same time it helped the community completing school, the main objective was to link women in society. Daily VAT on my aunt to the more intricate neighborhoods of Havana organizing events for incorporated to all women to the studio and work. Seeing as my aunt headed different projects was as he began to wake up inside of my attitude of leadership.

Continue as student of accounting because I had no options to change. I did a 10 student group which us meted to study and so push forward our career. During the four years of the same, I focused more on strategies to handle projects of my studio than in the content group to review. Each test and each work group is converted in a challenge for me, I organized that material came to study, where when and meted according to alumnus group was best for each of us. I was in charge of contacted different libraries for confirmed if they had the materials with which needed study. Went in groups and work towards the front of the class in this way presentations gave him another approach to my career since I enjoyed the leadership of my studie’s group.

In the year 2002 I graduated as Accounting. My first job was as a payroll clerk at the Department of accounting of the Municipal Directorate of Popular Power Old Havana where already spend a year of training as a student. My boss Gisella always remember her with great appreciation. Despite being my first job after graduating my boss put in my hands to create a program of payroll for the Department’s Foreign Agricultural Service. As from my practice is had managed what would be my tasks where the work would be in my. With my school experience most everything I learned during my practices to achieve create the. program With the same win me respect for all employees of the Department already a newly graduated with no wok experience been able to create and implement a new payroll system, which began using it in branch and later it was expanded and used for all company. As a Payroll Clerk my obligations were accountable for collecting timekeeping information, incorporating a variety of deductions into a periodic payroll, and issuing pay and pay-related information to employees there work till December of the same year. He had grown up in knowledge and was in search of better opportunities.

In October 2002 while working as a pay clerk roll pass a course of Actualization of Techniques of Sales to Sellers than they offered in the school of hospitality and tourism the Hotel Sevilla in Havana Cuba. The course gave me the possibility of working in tourism. In Cuba working with tourism is one of the opportunities not given twice, incredibly a tourism worker lives major than a doctor. My aunt changed work this time was Manager of a shop in a tourist company. I had the opportunity to the company as a cashier seller. First worked with the public my role was to attend to the public and to act will be Friendly with customers, responding their questions, plateful them locate items, and providing advice or references. Operating scanners, scales, cash registers. Accepting payments, confirming all prices and amounts are correct and verifying a receipt to all customer. It was an easy job as difficult was the hours worked 10 hours hours a day on stand up. The payment was good Clients always left tips. I was very good but it had already married me and my husband did not want to live in Cuba since the political-social and economic problems affect the town too much.
In 2003 I had the opportunity to leave Cuba for the Bahamas. I lived on several of its islands such as Nassau, Freeport, Bimini, Andros, Paradise Island and Kamalame Key. There I learned several of its cultural and religious aspects. During my years lived in the islands I dedicated myself to study their culture and religion, since I wanted to know the idiosyncrasy of the population where I was living. My studies were on my own I read many books and I studied internet articles I did not have the possibility to do it in a professional way.
In the year 2006 I came to live in the United States for me the best of the countries in the world, coming here was a dream come true, where I saw the possibility of growing as a professional and finally studying the career that would turn me on. The first thing I did was to visit the most serpent church of Saint Peter and Paul Catholic Church where I have been given my Support in religious services. Today I dedicate my free time to volunteer activities at Saint Peter and Paul Catholic Church as Food and clothes drives, Invest our community to learn how to build their resumes and job search on the computer, Blood pressure screenings for the elderly, Empower our community the importance about Cancer prevention, awareness, education and pathophysiology. Based on Faith sharing my knowledge with my community.

On February 2007, I had my first job at Publix Supermarket Meat Clerk Work as in the meat department were I was responsible for weighing, wrapping and pricing meat for customers. Working in Publix until 09, 2007 wasn’t anything relevant in my life I could analyze that it was a large company with wide possibilities of professional growth but felt that wasn’t in my industry.

In October of 2007 started to look for work in the newspaper this time wanted an industry that I step open in the country and give me at the same time opportunity of growth and focus in a field where I felt fascinated and passionate. I found an ad that I call attention “medical company seeks Customer Services, doesn’t need English”. I went to my job interview and is among those selected. The company offered service by phone belonging to a group patients medical. NC Brokers is your name where I Developed retention strategies resulting in membership increase. Customer feedback collected. and create process changes to exceed customer satisfaction. I felt comfortable in the work towards, for the first time I felt that was professional mind in the right industry.

At first I did not understand well what it was about but by nature I am not a friend of leaving things halfway. In September of the same year, I certified myself as Notary Public by State of Florida, maintained my certification and used my cuno with friends and family never for business purposes. I needed English then I enrolled in The English Center in the SAVES program and I studied English as a Second Language from 2009 until 2011. At the same time I needed an approach to start inquiring and in 2009 I began to study nursing assistant in La Esperanza Educational Services 2009 the course had several stages where I learned about OSHA Standards / TB / Infection Control, Biohazard Waste, Domestic Violence Seminar, Alzheimer’s Disease, and AIDS. The course had a part where I had to learn to give Cardio Pulmonary Resuscitation to patients if they needed it was where I understood that definitely my health is my world is my industry I felt in the right sense, but not the direct deal with the patient. I was passionate about an administrative career where I was a kind of Cybernetic Dr, where I could detect what illness a patient has and how to treat it without seeing it, without touching it, without curing it or medicating it.

Meanwhile, in April of 2011, they offered to take me from the call-back to one of the medical centers for which we offered customer services. Total Diagnostic Solutions the diagnostic center of IMC Health my job was Front Desk We were 2 girls I always despite not having the title I felt like the team leader my responsibilities were Carefully reviewed medical records for accuracy and completion as required by insurance companies. I had two electronic medical records Medisoft Billing System and Care Cloud Electronic. In my eagerness to overcome I sat with a counselor from the English center which recommended a course that would begin to impart medical billing and coding. I started my research and I was extremely attracted with the details of the Career in the same had classes like Basic Health Core-Basic Medical Knowledge, Medical Terminology, Anatomy and Physiology, Basic Health Core-Basic Medical Knowledge taught me to code and billiards with 3M Billing and Coding system. The important detail was that although you studied and approved all the classes you needed a national license issued by the American Academy of Professional Coders to work as a Medial Coder. At the end of the course I began to prepare myself to present myself for my first national exam, which approves and launched my professional career to 10 steps of advance.
In September of 2013 I applied to the same company for a Medical Coder position, after insist I was given the opportunity. In my world was one of my best moments as a professional could implement everything I had learned studied. My first Electronic Medical Record was MD Flow, which did not only bill it for codes patient diagnoses and treatments. My duties included Reading and analyzing patient records. Interacting with physicians and assistants to ensure accuracy. Over time I was appointed team leader of 2 medical centers and worked with 8 doctors to whom I reviewed the medical note to be coding compliance, and reviewed the medical charts in search of new opportunities. The Emr changed this time the company started working with MD vita and the base data used for the reports was Revenue max / Clear vision.
In order to maintain my licenses, the academy requires CEU annuals for which I have passed several courses as provided by the same academy. Which guide me when it comes to training doctors, administrative centers, medical assistance or new coders.
In 2014 I began to study the Certified Professional Medical Auditor course in Excelsior and thus persist to obtain the Medical Auditor's license in the field of coding one of the most interesting branches is the audit since you learn to examine medical records to ensure accuracy of billing and payments. Became experts in third-party payer, third-party payers usually being insurance companies or government agencies like Medicare, regulation to ensure compliance. At the same time in that year, take an update with the academy of "ICD-9-CM Official Guidelines Explained"
In October of 2015, CMS changed from ICD 9 to ICD 10 in order to comply with the course "ICD10 CM, ICD-9-CM Official Guidelines Explained along with Evaluation Chart Auditing". After both courses I had to take an exam and participate in a live online seminar to train me how to introduce the new guideline to the different providers.
2016 The academy offered CEU for the training "Modifier 25 - In Depth Analysis and Best Practice", which was extremely difficult for me since I learned in that way I could teach the doctors of the medical centers that I was leading at that time. Providers must be aware of the proper uses to safeguard themselves in case of carrier audits and MRA Education & Training Disease Specific, in which I learned that Medicare Risk Adjustment all providers need to report the chronic condition that each patient has if they do not tire out organization.

In the year 2017 I already had four years of Medical Coding experience and tree years being team leader, as a team leader I use all that I learn in my courses of American Academy of professional coders I learned to explain to the doctors how to report the chronic conditions that patients have we grow the company that when we raise the MRA score of the patients we generate profits and the payments received are what help us to pay for the services that patients really need, from the corporation my boss proposed me to go to work to the team of auditors where my function would be lead 3 clinics the center of Hialeah, Little Havana and Orlando would be auditing 10 doctors and 10 Medical Coders monthly, choosing three charts for each one and reviewing them at the same time distributing the work so that the team leader of the centers would distribute it to the coders. Every month I evaluated the work of the Doctors by checking 3 charts for each of them. On a monthly basis I visited the centers and monthly meetings and then report to the Manager and Director of the department. My work went well in the first evaluation I received as an auditor was excellent, I had an increase in salary and at the same time I reached the top of my position based on the standards of the company for which I worked.

 I keep abreast of each course approved by the academy of each update as I keep my providers updated. And in the 2017 Post-Election Policy Insights, I prepare for the 2018 updates.

At this time the programmers of the company together with my support create an internal data system called Semantix for all members with medicare we have options to see the demographic of the patient, all the previously reported chronic conditions and the documentation support, the MRA Score of each one of them, at the same time I can create reports with the conditions based on the result, medication list and hospitalization and utilization data. I work with Google G Suite through which I use Gmail for emails, Google Drive for storage and sharing files, Google Docs, Sheets and Slide are a word processor, a spreadsheet and a presentation program, Google Hangout for communication including text, voice and video conversation Google Calendar for schedules and remainders. Google Keep is a note-taking app. Each of the workers of my team has a special talent, my project is to discover it and my objective is to make it functional for the growth of the organization. Coca Cola monitors its self-service machines to collect data on the flavors that are on high demand and most preferred by consumers. Social media is another source of information for Coca Cola. They use these platforms to get feedback regarding their customers and release appropriate information that does not offend anyone. Most consumers base their purchasing decisions on the information they get from these platforms hence Coca Cola analyzes these platforms to find out what information people are attaching to their photos through computer vision. Coca-cola advertising records show that the company is the most popular producers of soft drinks. The use of a common logo, designs, and brand image has expanded its international popularity. The company advertises through various channels after analyzing their data. Some of their target markets get the information via the social media where the company has over 7 million followers, marketing directly to consumers, web-based media where their ads are added to customers who have seemed interested in the same, and promotions. From the analysis of the target market, the optimum customers can be obtained if the marketing strategies are used accordingly.

Despite all these appealing examples, the company has faults such as insufficient visualization of their operations. Organizations have realized the benefit of using technological techniques to establish data-driven objectives. An example of such a company is Coca Cola where the analyzed data is used to increase the number of customers they get and retain. The company is able to do this by creating platforms that provide an avenue for them to air out their opinions. The opinions from various customers on their preferences and dislikes are then analyzed by the company and later on adjusted to meet the needs of the consumer. The company uses the data driven strategies to understand the behavioral patterns of consumers when it comes to purchasing groceries. They are also able to analyze the interactions suppliers have with their customers hence giving the organization an upper hand when deciding on whether they need to make further changes or not. Advertising to the target audience ensures that the company maximizes their profits in every targeted location.

Amazon Fresh and Whole Foods use data obtained from the frequency of purchase of customers from previous operating periods to come up with the predicted patterns of customers when purchasing groceries. The analysis is also helpful when coming up with locations where the target market can be found hence advertisement strategies can be set to suit the needs of the target market.
